GENERAL INSTRUCTIONS
FOR SUBMITTING A MANUSCRIPT

The Cleveland Clinic Journal of Medicine is an indexed, peer-reviewed journal published monthly by Cleveland Clinic as part of its program of medical education. The Journal is designed to meet the continuing medical education needs of general internists, cardiologists, and other medical subspecialists. The focus of the Journal is the application of developments in diagnosis and treatment of disease to patients encountered in daily practice.

The CCJM does not accept original research or standard case reports. For more information on the type and format of specific series, see Instructions for Authors.

SELECTION OF TOPICS
Topics should be of interest to practicing internists. Because many CCJM articles are invited, please contact CCJM editorial staff to make sure that a similar article has not been invited.

SUBMISSION OF MANUSCRIPTS
Articles should be submitted via the Cleveland Clinic Journal of Medicine’s online manuscript submission and peer review system (www.editorialmanager.com/ccjm). A tutorial on how to use the system is available, but questions can also be sent to ccjm-em@ccf.org or 216-444-2661.

AUTHORSHIP BY RESIDENTS AND FELLOWS
Medical students, residents, and fellows may submit articles and may be listed as first author. However, CCJM policy requires that a senior physician mentor the junior author and be listed as a coauthor.

PARTICIPATION OF PHARMACEUTICAL AND MEDICAL EDUCATION COMPANIES
The CCJM does not accept articles written by authors who are employed by pharmaceutical companies, or papers prepared by medical education companies whose work is funded by pharmaceutical companies. COVER LETTER
Include a cover letter with the full name, address, telephone number, e-mail address, and fax number of the corresponding author. Authors will be sent a financial disclosure form after the paper is submitted, but if there are any special issues, please alert the editors in the cover letter.

MANUSCRIPT PREPARATION

Title Page
The title page should include all authors, their institutional affiliations, academic titles, and contact information.

Formatting
Text should be typed and double spaced, in 12 point type. Although a variety of formats are acceptable, Word is preferred. Please note that tables and figures should not be imbedded in the text of the article.

References
Number references in the order in which they are cited in the text. Follow the Uniform Requirements for Manuscripts Submitted to Biomedical Journals (http://www.icmje.org). Abbreviate periodicals according to Index Medicus. If a citation has six or fewer authors, list all authors; if a citation has seven or more authors, list the first three, then "et al." Authors are responsible for the accuracy of references.

Tables and Figures
Tables and figures should not be imbedded in the text of the article. Where photographs of patients are included, consent to publish the photograph, signed by the patient, must accompany the manuscript. Drawings, graphs, and charts should be professionally rendered. Copyright permissions for tables and figures previously published
If a table or figure has been published elsewhere, provide a permission letter (for both print and electronic publication) from the publisher, even if it is from the author's own work.

PEER REVIEW
All manuscripts are peer reviewed usually by at least one content expert and general internist. Authors are usually notified within 6 weeks about the acceptability of a manuscript, but longer intervals are sometimes unavoidable.

EDITING AND GALLEY PROOFS
All papers accepted for publication will be edited to conform to the Cleveland Clinic Journal of Medicine editorial style. Authors are responsible for all statements made in their work, including any changes made by the copy editor and authorized by the corresponding author. Copyright ownership must be transferred to the publisher. An appropriate form will be supplied.
